Checklist For Required Proposal Materials

( ) One page cover letter outlining nature of the project

( ) Proposal, not to exceed six pages, including description of project and
where applicable, implementation and assessment plans

( ) One page budget including a time-line of activities

( ) Two copies of all completed application materials sent in one
package, postmarked by October 1st to:
